When joining the club swimmers need to fill out the <a href=\"..\/..\/wp-content\/uploads\/2025\/07\/2025-Membership-Form.pdf\">membership form<\/a>, <a href=\"..\/..\/wp-content\/uploads\/2025\/07\/2025-Medical-Information-Form.pdf\">medical form<\/a> and the appropriate code of conduct form(s) and agree to abide by the club\u2019s terms and conditions. The code of conduct form(s) required are:<\/p>\n<ul>\n<li><a href=\"..\/..\/wp-content\/uploads\/2025\/07\/2025-Code-of-Conduct-Member.pdf\">Code of Conduct Form (Member)<\/a> for each swimmer<\/li>\n<li><a href=\"..\/..\/wp-content\/uploads\/2025\/07\/2025-Code-of-Conduct-Parent.pdf\">Code of Conduct Form (Parent)<\/a> for each family with swimmers under-18<\/li>\n<li><a href=\"..\/..\/wp-content\/uploads\/2025\/07\/2025-Code-of-Conduct-Teacher.pdf\">Code of Conduct Form (Teacher)<\/a> for each teacher or coach<\/li>\n<li><a href=\"..\/..\/wp-content\/uploads\/2025\/07\/2025-Code-of-Conduct-Volunteer.pdf\">Code of Conduct Form (Volunteer)<\/a> for each volunteer<\/li>\n<\/ul>\n<p>Gift Aid raises more funds for Mildenhall &amp; District Swimming Club without costing you a penny more. If you are a UK tax payer we can reclaim the tax on membership subscriptions. For every \u00a31 of membership fees and donations a further 25p can be claimed from the Inland Revenue.
