The welcome pack contains information on the classes offered, membership fees, training times and code of conduct. When joining the club swimmers need to fill out the membership form, medical form and the appropriate code of conduct form(s) and agree to abide by the club’s terms and conditions. The code of conduct form(s) required are:
- Code of Conduct Form (Member) for each swimmer
- Code of Conduct Form (Parent) for each family with swimmers under-18
- Code of Conduct Form (Teacher) for each teacher or coach
- Code of Conduct Form (Volunteer) for each volunteer
Gift Aid raises more funds for Mildenhall & District Swimming Club without costing you a penny more. If you are a UK tax payer we can reclaim the tax on membership subscriptions. For every £1 of membership fees and donations a further 25p can be claimed from the Inland Revenue. Please fill in a Gift-Aid Declaration form if you are able to gift-aid your membership subscriptions.
